Cancellation Policy

If you need to cancel an appointment, we require 24 hours notice, or a **$40** fee will be charged for a missed appointment.

Missed Chronic Disease Management, Pap test, driver's medicals, and other appointments bookef for 30 minutes or longer will be charged a **$80** fee.

Three (3) "No Show" appointments will result in termination of care for the particular patient.

Breaking appointments prevent us from taking care of other patients in a timely manner. Thank you for your consideration.
