Whether you have not been feeling yourself lately, or your doctor sent you to complete one of these forms, these questionnaires can be quite helpful in shedding light on your mental health. It is important to note, these tests are not diagnostic. They are being used here as screening tools, and need to be interpreted by your doctor in conjunction with your other symptoms and concerns. Talk to your doctor about your results - print these forms and bring them in with your appointment to help you start the discussion.
